Case Management at the Ryan Center is designed to connect you with the fullest range of necessary services by assessing your individual needs, establishing goals, and providing access to relevant services. A Case Manager meets with each newly registered patient and establishes a working relationship.

The Case Managers at Ryan assist their clients through a process which respects their dignity, and aims to strengthen their individual support network through difficult times. The Case Managers are experienced, knowledgeable and involved professionals who provide advocacy, special support, service coordination, and educational services to their clients. Assistance in finding new and suitable housing, obtaining insurance and public assistance benefits, and being reunited with their families through work with the foster case system is also provided as needed.

In addition, the experienced care technicians with Ryan’s Maintenance in Care Program can help you stay connected to care by providing you with home visits, escorts to medical appointments, and health education on treatment regimes.
